Elk Ridge Community Health exists to provide accessible, high-quality, and compassionate healthcare to the communities of northern Arizona. As a Federally Qualified Health Center (FQHC), we are committed to serving all patients — regardless of their ability to pay.
Our sliding fee scale ensures that no one is turned away due to financial hardship. We offer an array of primary care services at 12 locations across northern Arizona.

Federal Designation
FQHC & FTCA Designation
Elk Ridge Community Health is a Federally Qualified Health Center (FQHC) — a designation that reflects our commitment to providing comprehensive, cost-effective primary care services to underserved communities.
As a deemed Public Health Service (PHS) employee under the Federally Supported Health Centers Assistance Act (FSCHAA), our providers are covered by the Federal Tort Claims Act (FTCA) for medical malpractice. This means our patients and providers are protected under federal law.
This designation also enables us to offer the 340B Drug Pricing Program, allowing us to provide medications at significantly reduced costs to eligible patients.

Federal malpractice protection for all clinical providers under the Federal Tort Claims Act.
